Cast iron grill pans have the same material safety profile as any cast iron cookware: no synthetic coatings, no PFAS, no off-gassing at any cooking temperature. The raised ridge pattern is seasoned iron; the valleys where food sits are seasoned iron. Everything is the same material, just shaped differently.
The ridged surface creates a maintenance consideration. Fats, food particles, and carbonized residue accumulate in the channels between ridges. If not cleaned thoroughly, this build-up can become a harbor for bacteria over time and can affect flavor. A stiff brush (copper, nylon, or a chain mail scrubber) works better than a flat sponge for getting into the ridges. Hot water and a brief scrub after each use, followed by drying on the stovetop and a thin oil coating, keeps the surface in good condition.
Smoke is worth considering. At the high heat suitable for good grill marks — 450°F+ — fat dripping onto the cast iron ridges smokes significantly. Adequate kitchen ventilation is important, both for practical comfort and to avoid triggering smoke alarms. A range hood on high or open windows makes the experience much more pleasant. Non-stick grill pans (PTFE or ceramic coated) are tempting for their easy cleanup, but the combination of ridged surface (harder to clean thoroughly) and high heat (exactly where coatings fail fastest) makes them poor choices long-term.
